When requesting a new business partner setup, please email support@custodia.ai and include the following information:

1. Card Types (yes or no)
   1. Hybrid
   2. Zero
   3. Pre-existing
2. Card Type names (for each selected in the previous question provide names for the cards)
3. Card Product Code
4. Card Network
   1. Mastercard
   2. Visa
5. Card Usage
   1. Physical
   2. Virtual
   3. Both
6. Sale Offer Code
